The Safety Incident and Near Miss Report is internal to the Department of Chemistry and is used to improve the safety of our students, faculty, staff, and visitors by identifying common sources of incidents and ways to reduce the risk of recurrence, not to penalize. While we may notify Environmental Health and Safety, the Institutional Biosafety Committee, or other groups of certain incidents, if you believe that your incident has mandated reporting requirements, you should also submit a report directly to those groups following their directions.
